Dear Patient, 

We are writing to you as you are a patient registered at Gravesend Medical Centre and Parrock Street Surgery, and we have important information to share with you. 

Earlier in 2025 Gravesend Medical Practice took over the running of the Parrock Street practice, and patients have been seen at our New Swan Yard site. We are formally merging the systems that we use to manage patient records, and we would like to take the opportunity to ask you about how appointments are managed.

We are reviewing how our appointment system works so that we can make it easier for patients to get the help they need, in the most appropriate way-whether that is face-to-face, by telephone or with another member of our clinical team. 

To help us understand what is working well and what could be improved, we are asking patients to complete a short survey about their experience of requesting and managing appointments, including how you contact us, and how clear the next steps feel. 

• A patient survey can be accessed below, or paper copies will be available at Reception at our New Swan Yard site.

• Our patient participation group (PPG) are supporting us and helping to reach out into our community groups for feedback. 

The engagement period will close on Sunday 15th March 2026.

Your feedback will be used to help shape improvements to our appointment system. Thank you for taking the time to share your experience-your feedback will help us to make access to care as straightforward and fair as possible.
